Self-care Deficit related to Decreased Ability and Self-care Motivation


Nursing Diagnosis :

Self-care deficit: personal hygiene and clothing / makeup r / t decreased ability
and self-care motivation.

General goals :
The client can increase the interest or motivation and maintaining personal hygiene.

Specific goals :

1. The client can build a trusting relationship.

Outcomes :
  • The client can express feelings and the current state:
  • Friendly facial expression.
  • Show pleasure.
  • There is eye contact.
  • Want to shake hands.
  • Want to mention names.
  • Want to answer greeting.
  • Clients want to sit side by side with nurses.
  • Want to express the problems encountered.
Interventions :
  • Create a trusting relationship by using therapeutic communication principles:
  • Greetings to the client with a friendly verbal and non-verbal.
  • Introduce yourself politely.
  • Ask the client's full name and nickname that the client prefer.
  • Explain the purpose of the interaction.
  • Honest and keep promises.
  • Show empathy.
  • Pay attention to the basic needs of the client.


Specific goals :

2. The client may know about the importance of personal hygiene.

Outcomes :

1. The client can mention the importance of personal hygiene within 2 meetings:
  • Signs clean.
  • There is no body odor.
  • Hair neat, clean and did not smell.
  • Teeth clean and no bad breath.
  • Neat clothes and no odor.
Interventions :
  • Discuss with the client the importance of hygiene in a way to explain the meaning clean and signs clean.
  • Encourage the client to mention 3 of 5 signs of personal hygiene.
2. The client is able to mention the hygiene returned to health, namely:
  • Prevent disease.
  • Gives a feeling fresh and comfortable.
  • Preventing damage to the teeth and maintaining oral hygiene.
Interventions :
  • Discuss the function of personal hygiene to health by reviewing the client's knowledge on matters relating to personal hygiene.
  • Help the client to express the meaning of personal hygiene and the purpose of maintaining personal hygiene.
  • Give positive reinforcment after the client expresses the sense of personal hygiene.
3. The client can explain how to care for themselves, among other things:
  • Shower 2 times a day with soap.
  • Brushing teeth at least 2 times a day after meals will sleep.
  • Wash your hair 2-3 times a week.
  • Cut the nails when the length.
  • Wash hands before and after meals.
Interventions :
  • Remind clients to maintain personal hygiene such as:
  • Shower 2 times morning and afternoon.
  • Brush your teeth at least 2 times a day (after meals and going to sleep).
  • Shampooing and combing hair.
  • Nail clippers when the length.
